Breast augmentation is often framed as a single decision made on a single day. In reality, silicone gel implants come with a long tail of maintenance that many Los Angeles patients only hear about in passing during a consultation. The most important piece of that tail is surveillance for silent rupture, a failure of the implant shell that produces no pain, no visible change, and no obvious symptom. This article explains what the current screening guidance says, why it changed in 2020, and what the two main imaging options actually cost in the Los Angeles market.

Why silicone ruptures can be silent. Modern silicone implants are filled with cohesive gel, meaning the silicone holds together rather than flowing freely. When the shell fails, the gel frequently stays inside the scar capsule the body builds around every implant. This is called an intracapsular rupture, and because nothing migrates and nothing deflates, the breast can look and feel completely normal. Saline implants behave differently: a ruptured saline implant deflates visibly within days because the body absorbs the salt water. That mechanical difference is the entire reason silicone patients need imaging and saline patients generally do not.

What the FDA currently recommends. In 2020 the FDA updated its labeling guidance for silicone gel implants. The current recommendation is a first screening at five to six years after surgery, then repeat screening every two to three years after that. This replaced the older schedule, which called for MRI at three years and every two years thereafter. The update did two notable things: it pushed the first scan later, reflecting data showing rupture rates are low in the early years, and it formally added high resolution ultrasound as an acceptable first line screening tool alongside MRI.

MRI vs ultrasound, mechanistically. MRI without contrast remains the most sensitive test for detecting intracapsular rupture, with published sensitivity generally in the range of 80 to 90 percent. Radiologists look for specific signs, most famously the linguine sign, in which the collapsed shell floats in folds within the gel. Ultrasound is faster, cheaper, and involves no enclosed machine, but it is more operator dependent. In experienced hands, ultrasound performs reasonably well, and its main role under the new guidance is as a screening gatekeeper: if the ultrasound is equivocal or suggests rupture, MRI follows for confirmation. If the ultrasound is clearly normal in an asymptomatic patient, many radiologists consider that sufficient for the screening interval.

What this costs in Los Angeles. Insurance rarely covers surveillance imaging for cosmetic implants, so most patients pay cash. In the Los Angeles market, a non contrast breast MRI at an independent imaging center typically runs $400 to $1,200 cash pay, with hospital based imaging often higher. High resolution ultrasound of the implants generally runs $150 to $400. Some plastic surgery practices in the region have added in office ultrasound and offer it at follow-up visits, sometimes bundled or discounted for their own surgical patients. Over a typical 15 to 20 year implant lifespan, a patient following the schedule strictly could undergo five or more screening studies, so the cumulative cost difference between the two modalities is real money and worth asking about before surgery, not after.

How compliance actually looks. Published studies and registry data have repeatedly shown that only a minority of patients follow the screening schedule, historically well under 30 percent under the old MRI only guidance. Cost, inconvenience, and simple lack of awareness were the main reasons. The addition of ultrasound was partly an attempt to lower those barriers. There is no penalty for skipping scans, and an undetected intracapsular rupture is not an emergency, but a ruptured implant left in place for years can progress to extracapsular rupture, where gel escapes the capsule and can migrate into surrounding tissue, making eventual removal more complicated.

Practical questions to ask before your Los Angeles augmentation. First, ask whether the practice offers in office ultrasound screening and what it charges. Second, ask which imaging centers in the area the surgeon refers to for cash pay MRI, since prices vary widely across the county. Third, confirm you will receive the implant device card with your implant style, size, and serial numbers, because radiologists use that information when interpreting scans. Finally, ask how the office handles long term follow-up if the surgeon retires or you move, since implants routinely outlast the surgical relationship.

The bottom line. Silicone implants are well studied devices, and rupture in the first several years is uncommon. But because rupture is usually silent, imaging is the only reliable way to know an implant's status. The 2020 guidance made surveillance cheaper and more accessible by legitimizing ultrasound, and Los Angeles has no shortage of imaging capacity. Budgeting for periodic screening from day one turns a surprise expense into a predictable line item, which is exactly how a long term medical device should be managed.
